• Ask us to limit what we use or share: You can request restrictions on certain uses or disclosures, though we are not always required to agree.
• Get a list of disclosures: You can request a list of the times we've shared your PHI (excluding those for treatment, payment, or healthcare operations).
• Choose someone to act for you: If someone has legal authority to act on your behalf, we will honor their decisions for your health information.
• File a complaint: If you feel your privacy rights have been violated, you can file a complaint with us or with the U.S. Department of Health and Human Services.
